By now, you should have received an e-mail from the Field Education office indicating that your application was complete and approved. If you have not received an acceptance e-mail, please contact field.coordinator@waldenu.edu.

This week, you’re ready to take the next step and submit your agency of choice on the Lead Agency Form in the Meditrek system.

Assignment: Field Agency Submission

After your Field Application is approved, you submit an Agency of Choice in Meditrek. The submission of your Agency of Choice is another key step on your MSW journey.

For this Assignment, follow the instructions below to confirm with your Instructor that you have made your submission.

By Day 7

• Log in to Meditrek and submit your Lead Agency Form.
• Submit a document here in your online classroom that confirms that you submitted your Agency of Choice. To do this, create your own word processing document and type the following information:
  • The date you submitted
  • A confirmation statement to your Instructor that states the name of the agency
• If you have encountered difficulty, state this in your document to inform your Instructor, and then do the following:
  • Contact field.coordinator@waldenu.edu.
• Locate information about applying for Field at the Field Education website at http://academicguides.waldenu.edu/fieldexperience/sosw.

Submission and Grading Information

To submit your completed Assignment for review and grading, do the following:

• Please save your Assignment using the naming convention “WK5Assgn+last name+first initial.(extension)” as the name.
• Click the Week 5 Assignment link.
• Next, from the Attach File area, click on the Browse My Computer button. Find the document you saved as “WK5Assgn+last name+first initial.(extension)” and click Open.
• If applicable: From the Plagiarism Tools area, click the checkbox for I agree to submit my paper(s) to the Global Reference Database.
• Click on the Submit button to complete your submission.
